  3H Fund provides grants to disabled people and their families on low income, to assist them in organising a modest holiday for themselves in the UK.   

Grant criteria : The holiday must be taken in the UK and you organise your own break to meet your needs. We pay the holiday company directly. Applicants should bear in mind that grants are for an individual or family group, not per person, and are usually in the region of £150 - £350 per award (the lower end of the range for an individual). We require recent evidence of low income and disability. We do not accept DLA for either form of evidence.

 

Applications must be made by filling in an application form which will have a grant number in the top right hand corner.  Please make a note of this number and quote it in all future correspondence as it helps us to identify your application. Applications may be made by the disabled person, by a member of their family or by a referring agency such as Social Services.

 

Funds are restricted and grants are awarded at the discretion of the Trustees, therefore there is no guarantee that your application will be successful and holidays should not be booked until confirmation of a grant has been received.  Grants are not given for holidays already paid for.  If a holiday has already been booked it may be assumed that funding has already been found for the holiday and therefore a grant will not be awarded.

 

To assist our Trustees in assessing grant applications, the following information is required:

·         The name, address, telephone number and date of birth of the disabled person

·         The name, address and telephone number of the applicant, if different to the disabled person (i.e. social worker, parent etc)

·         Recent evidence of disability. (Not DLA) This can be a letter from a GP, Hospital, Social Worker etc ·         Recent evidence of low income. (Not DLA) This can be a copy of a letter showing income support etc

·         Date and details of holiday - If known.

·         Number of people going on the holiday and breakdown of adults/children

 

We also need to know:

  • Number of people going on the holiday. 
For our reporting purposes, we need to know how many people will benefit from a grant.
  • Total cost of holiday and amount of grant applied for. 
There is no penalty for applying for the total cost of the holiday (this does not include spending money or travel).  If the amount awarded is less than the cost of the holiday, the applicant must make provision for paying the difference.
  • Supporting evidence. 
Any further evidence in support of an application will help the assessment process and can be detailed on the space provided and on an additional sheet of paper if necessary.     If an application form is not returned by the date indicated in the accompanying letter, the application will be cancelled.  Grants promised must be accepted by telephone, email or letter and arrangements can then be made for payment.  If a grant is not accepted within a reasonable time limit, the grant offer may be withdrawn.  Holidays must be taken within the grant programme year.  It is the responsibility of the applicant to remind 3H Fund when payment becomes due and payment is generally made direct to the Holiday Company.                    GRANTS MUST BE PAID FOR BY 31.10.09 AND HOLIDAYS TAKEN BY 31.12.09     Please note that only one grant may be awarded to an individual/family within a grant programme year and a period of at least 9 months must have elapsed since the last holiday paid for by a grant has been taken. If you have had 2 consecutive grants or more, priority may be given to new applicants.

The information provided by me to the 3 H Fund whether on this application form, by phone, or otherwise is accurate and not misleading.

I agree to only use the 3 H Fund grant for the purpose given.

I understand that help from the 3 H Fund is discretionary and subject to funding. There is no entitlement to a grant and all cases are assessed on an individual basis.

The 3 H Fund may require that I agree to be bound by additional terms and conditions before specific grants are made.
